A conversation between a patient and a doctor about the registration forms
Patient:
When I go to hospital, I will be given a registration form if it is the initial visit. I am a little tense up when I have to fill the form because I don’t understand all of questions written in English.
年末年始は家族揃って海外旅行などに出かける絶好のチャンスです。しかし、短い滞在であっても、けがや腹痛、あるいは持病が出るなどして、地元のお医者さんのお世話になることもめずらしくありません。病院の受付で、手続きの際の質問事項や予診でのやりとりに、少しでも正確に答えたいと思うのですが. . . .
Doctor:
I understand your feeling, but you don’t have to think about it too seriously. Basic contents are the same as the ones you write for hospitals in Japan. If there are items you are unsure how to answer, you can leave them blank and tell the receptionist. When you are called by name and proceed to go inside, nurses or medical assistants will measure your body weight, body temperature, blood pressure and pulse. This is called preliminary examination. Body temperature in the US is measured by inserting a thermometer under the tongue, not in the axilla as done in Japan. When the thermometer is inserted under the tongue, hold it lightly between lips. An electronic thermometer will take only 10 seconds or so.
お気持ちはわかりますが、そんなに難しく考えなくても大丈夫ですよ。基本的な内容は日本の病院の手続き用紙（Registration Form)とそう変わらないと思ってよいでしょう。地元の保険証をお持ちでししたら、あらかじめ提出なさっておくと、手続きが早く済みますし、旅行保険をお持ちの方も提示なさっておくと、必要な書類(例えば費用の払い戻しを受ける場合)をもらう際の説明を省くことができるでしょう。登録用紙には、わかる事項だけ記入して、受付に提出されて結構です。向こうから尋ねてくれますから、それにあわてすに答えて下さい。呼ばれて中に入ると、まず、予診といった形で看護師さんや助手の人が体温、体重、血圧、脈拍などを測定します。ただ、体温測定は腋下でなく、舌下で行われます。軽く唇で固定しておきますと、現在使われている電子体温計では約１0秒で終わります。
In the US, Fahrenheit (°F), not Celcius (°C), is used to express temperature. It is recommended, therefore, to know your usual body temperature in°F. Similarly, pound (lb), not kilogram (kg), is used for body weight.
温度は摂氏（℃）ではなく華氏( °F）ですので、ご自分の平常体温を華氏で覚えておきましょう。それから体重はキログラムでなくポンドが使われます。

持病のある方、薬を常用しなければならない方は、病名や薬の名前を英語で予め知っておかれると助かります。紙に書き留めておかれてはどうでしょうか？
