A conversation between a patient and a doctor about prostate cancer
Patient:
The latest news reported a decrease in the mortality due to prostate cancer. It may be related to the decrease. I hear that prostate cancer can be detected by blood tests. How does it work? And, explain briefly about the prostate, too.
最近の報告では、前立腺癌死亡率の減少が伝えられていますね。それに関係あることでしょうが、血液検査で、前立腺癌が発見されるということですが、どういうことですか。前立腺についても簡単に説明してください。
Doctor:
The prostate is situated right below the bladder in the male, and surrounds the urethra. The prostate stores and secrets prostatic fluid which helps prolong the lifespan of sperm. The smooth muscles contained in the prostate help expel semen during ejaculation. The prostate enlarges as men age. Half of all men 75 years or older are said to have prostate hypertrophy (enlarged prostate). When the enlargement affects urination, it is called benign prostatic hyperplasia (BPH)．If you have any difficulty in urination, including a feeling that the bladder is not empty, or frequent urination, consult a urologist. A regular check-up is also important. The prostate specific antigen (PSA), detected in blood test, is the new tool for the screening of prostate cancer. However, the validity of PSA has not been established. Although the PSA may not be the major factor for decreasing in the mortality due to prostate cancer, it has certainly contributed to men’s increased participation in the screening for prostate cancer.
前立腺は、男性の膀胱から尿道へ移行する部位にあり、尿道をぐるりと、とり巻いています。前立腺は前立腺液を分泌しますが、この分泌液は精子の寿命を延長します。前立腺に備わっている平滑筋は射精における収縮の働きをします。前立腺は加齢にしたがって肥大し、７５才以上の男性の半数は前立腺肥大があるといわれています。前立腺の肥大が排尿に影響する場合、良性前立腺肥大とよばれます。残尿感、瀕尿感、その他の排尿障害を覚える方は、泌尿器科医 (urologist) の診察を受けることが大切です。又、定期的健康診断も、ぜひ実行してください。血液検査で検出するprostate specific antigen（PSA, 前立腺に特殊な抗原）は、前立腺癌のスクリーニングの新手ですが、その確実性は、現時点では確立していません。PSAは前立腺癌の死亡率の低下の主因ではないようですが、スクリーニングを受ける人の増加にはPSAが寄与しているのは確かです。
